Flu Vaccine
Flu shots will be available at the Alta Community Center on Thursday, October 17th from noon to 2:00pm.
Forms can be filled out on-site, but you can also print the consent form, fill it out beforehand, and take it with you. The consent form can be found on our website. www.altaiowa.org
